What should I expect when I arrive for my appointment?

Arrival time

Your appointment time is reserved just for you. We ask that you arrive 15 minutes before your scheduled appointment to complete a short registration process.

Late arrival

Our offices make every effort to stay on schedule. If you do not arrive on or before your scheduled appointment time, it may be necessary to reschedule your appointment.

Cancellations

If you cannot keep your appointment, please call our office immediately to cancel. Please call us at least one full business day prior to your scheduled appointment to avoid fees and patient dismissal, if possible.

What should I bring to my appointment?

Please bring all your current prescribed medications with you to each appointment.As a patient, you are responsible for providing current insurance information (health insurance card or a photocopy of it) at the time of service. If your insurance cannot be verified at the time of the appointment, you will be asked to pay in full for the visit at the time of check-in or asked to reschedule.

Co-pays and co-insurances are the responsibility of the patient and are expected at the time of service.

We view healthcare as a partnership between you, your physician, and our office. Please see our Patient-Provider Agreement for more details.

If you have a MyChart account, you may also cancel your appointment online or through the MyChart app.

Vaccination policy

Because we are committed to protecting the health of your family, we require all of our patients 18 years of age and younger be up-to-date on recommended vaccines in order to foster a safe, healthy environment at our medical clinics.

We firmly believe in the effectiveness of vaccines to prevent serious illness and to save lives. CMU Health follows the Michigan Department of Health and Human Services vaccine requirements for child care and preschool and requirements for school entry . Patients must comply with the recommended schedule for vaccinations to remain a part of the CMU Health network.

Patient privacy

CMU Health clinics provide patients with an electronic medical record that allows doctors and nurses to access medical history, patient instructions, lab results and more during an appointment. This technology improves the efficiency of the appointment, and the system ensures patient privacy and confidentiality.
